This Notice of Privacy Practices, mandated by the Health Insurance Portability and Accountability Act of 1996 ("HIPAA"), describes how Omnisyn Health LLC, Omnisyn LLC, and the Omnisyn Network, collectively referred to as “Omnisyn,” may use and disclose your protected health information (PHI) for treatment, payment, and healthcare operations, and other purposes permitted by law. PHI includes information about your past, present, or future physical or mental health, related care services, and payment for services.

We are required to abide by this notice and may revise it at any time. Any changes will apply to all PHI we maintain. You can request the latest version at any time by contacting us. By using Omnisyn's services, you acknowledge receipt of this notice.

1. Uses and Disclosures of Protected Health Information

Your PHI may be used and disclosed by Omnisyn for treatment, payment, and healthcare operations:

- Payment: To obtain payment for healthcare services provided to you.

- Healthcare Operations: For business activities like quality assessments, employee training, and provider licensing. We may share your PHI with "business associates" who perform services like billing and care coordination for us, always with safeguards in place to protect your privacy.

2. Other Permitted Uses and Disclosures Without Your Consent

We may also disclose your PHI without your consent as required by law, for public health activities, to prevent a threat to public safety, for health oversight, in legal proceedings, and in other circumstances outlined by HIPAA regulations.

3. Your Rights Regarding PHI

You have the right to:

- Inspect and obtain a copy of your PHI.

- Request restrictions on how we use or disclose your PHI.

- Request confidential communications.

- Amend your PHI.

- Receive an accounting of certain disclosures of your PHI.

You may communicate with Omnisyn electronically, but please note that email is not always a secure method of communication. By choosing to communicate this way, you accept the risks of unencrypted data transmission.

5. Complaints

If you believe your privacy rights have been violated, you may file a complaint with us or the Secretary of Health and Human Services. We will not retaliate against you for filing a complaint.
